TEIDe’s power lies in how quickly its science can reach the clinic. Using epidemiological data from more than three million adults in the Dementias Platform UK, the consortium is building an easy-to-interpret risk-score that family physicians can run in seconds to flag patients who warrant early prevention efforts, the engine behind the forthcoming DemAware mobile and EHR plug-in tool . Once a patient is identified, TEIDe’s precision-medicine work bundles the evidence from eight rigorously controlled trials into algorithms that pick the “right” dose and type of exercise for each individual; the recommendation is delivered through concise manuals and the future MoveMentor app so that a general practitioner can issue a personalised prescription during a routine ten-minute visit.

The project then tests the full workflow in the real world. A 12-week pilot at El Palo Primary Health Centre (Andalusia, Spain) will (i) screen roughly 415 callers, (ii) invite 60 at-risk adults into care, and (iii) run small-group exercise sessions led by an exercise-science professional inside the clinic . Feasibility metrics such as 75 % adherence, safety events and staff workload are built in, while patient and caregiver voices shape every step through dedicated focus-groups. The goal is to prove that dementia-prevention workflows can fit seamlessly into day-to-day primary care, just as blood-pressure or diabetes checks do today.

By year 3 the consortium will hand over four concrete, practice-ready assets: validated dementia risk-scores, AI-driven exercise-prescription algorithms, clinician-friendly exercise manuals, and an open implementation playbook for health systems . These outputs pave the way for guideline updates at WHO and national levels and for rapid roll-out of the DemAware and MoveMentor apps in other countries, marking a decisive shift from reactive dementia care to proactive brain-health promotion.
